I think this is self explanatory. I tried to create a 0.125 offset on the outside of my object. What did I screw up?

Long story short - I think that is a bug!
I tested your drawing and figured out that if I use an Offset value (e.g. 0.064) which is less than the length value of this little vertical line - it will work as expected.
A value higher than the length of this vertical line (e.g. 0.125) will fail.
I also tested this behavior on a new drawing - same result!

The current algorithm does not support that kind of shape.
Long term, QCAD might switch to an alternative algorithm altogether, possibly something based on an existing library.
