From a QCAD user:

The crosshairs mouse cursor is not at the correct position or the mouse cursor is not visible at all for some functions or the mouse cursor does not have the desired size.

There is a problem somewhere between Qt (the toolkit QCAD uses) and your system with custom mouse cursors.

You can force QCAD to only use system cursors by enabling the preference:
Edit > Application Preferences > Graphics View > Appearance > Use system cursors
