I'm not sure if this is a thing with PDFs but is there a resolution when using the "PDF Export" feature ?
I've been having problems trying to print to a PDF device (using the printing feature with my PDF driver and not "PDF Export"). The lines always ended up with different weights, even though they were all the same in QCAD. The text didn't scale properly either. Long story short, I realized that my PDF print driver was using a default of 300dpi. Increasing it to 1200 resolved the issues I was having.
The file size using "PDF Export" is about 10% larger in size than the file size printing to 1200 resolution.

QCAD export to pdf is in vector graphics.
The difference in quality with a pdf printer device should be very clear when zooming in with a pdf viewer.
