Given: Template with prepared Paper Space Frame/Titleblock scale = 1:1.
Drawing 1:1
First Viewport / scale 1:1 / inserted in Paper Space = everything fine,
Second Viewport / scale 0.02 will scale the Paper respective to the new Viewport scale but the Frame/Titleblock isn't scaled.

Unfortunately, I don't follow.
Maybe you can you put this into a list of steps that is easier to understand for me:
1. File > New From Template
2. Choose template "Viewport Scale issue"
3. ...
4. ...

Just a hunch: could it be that you are accidentally changing the scale of the paper in the print preview instead of the scale of the viewport (note there are two scale combo boxes in the options tool bar)?
Yes - you are right. My bad - sorry for that! :(
My focus was on the left scale combo box which appears in the first dialog (creating the Viewport) but the next dialog is the combination of Print Preview + scale for the Viewport which is then on the right side ...
I guess I've to calibrate my eyes a bit better .... LOL ...
